Power Steering Pump or Steering Rack?
Started out as an intermittent problem, but now has gotten to near constant.
When I'm standing still and I try and turn the wheel, I got very little power assist, and my pump belt starts screaming. I've tightened the belt and it still does it.
When i turn the wheel, I occasionally get a loud *thunk* from the front of the car.
When I'm moving it's not nearly as bad, but still happens.
And every now and then, I will lose ALL power assist, with no audible symptoms.

You should never turn the wheel while stationary.
Are you sure you've tensioned the belt correctly? it ought not be slipping no matter what sort of problem you have (unless the pump is seizing lol.. but that REALLY would make a bit of a squeel lol)
i know it sounds silly, but i tensioned mine completely wrong for a year or so lol.. untill you can get under the car and have a propper look its hard to see quite how the stupid thing works..
as for the clunk.. not so good... get the front wheels up in the air if possible and try the rack then, see if it has any stiff points or if the clunk is there.. sounds like a balljoint or something though maybe, or top strutt mount? check for play in the suspension turrets and balljoints etc by giving the wheels a good twist and turn every which way you can
